Becoming a Patient
Most patients come because of a doctor's referral or a referral from a friend or family member who was a patient here. Either way, we are glad you are here! Please let us know who we can thank for referring you to us. There are a few things you should know before you come in for your first visit. In California, you have to have a prescription or a diagnosis from a doctor before you can be treated. You must have a prescription if you want your health insurance to pay. If you have any questions about insurance, have your insurance card available when you call the office and we will be happy to help you. You may want to visit our Experience page to learn more about what to expect on your first visit. It will save time if you fill out and print the Patient Intake Form below before your first visit. If you have low back pain, please print and complete the Oswestry form also.

In order to save time, you may want to complete one or more of the three forms below before coming in to the office for your first appointment. Click on the form to open it up in Acrobat Reader. You can then type your answers into the first two forms and print out the forms with your answers typed or you can print the form and complete in by hand. For the Oswestry, just print it out and circle the appropriate numbers and bring it with you.
Patient Intake Form - Use this form if you already have a prescription for therapy from your doctor.
